Sub: tendering my resignation – Reg.

With all due respect, I would like to inform you that I tender my resignation. I have already been in medical leave for the past two months due to lower back pain and neck pain. My doctor has advised me to be on complete bed rest and I am physically unfit to discharge my duties.  On account of this reason, having no other choice, I am resigning my job. I have performed my duties true to my conscience all through my tenure. I humbly request you to accept my resignation considering my physical ailment and relieve me from all my duties.

I take this opportunity to thank you wholeheartedly for having given me such a wonderful chance to work with a great banking empire.
