With reference and reply to the letter dated 17/03/2022 stating the declining of my leave requisition, I write this letter.

The reason for my leave requisition is genuine and I am suffering from severe back pain. I am advised by a doctor not to travel so as to cure the ailment. I have attached doctor certificate to substantiate my cause. I humbly request you to consider my health issue and sanction the leave which I have requested.
