Sub: Requisition for medical leave - Reg.
    I have been suffering from lower back pain for the past two weeks. I consulted a doctor and he has advised me to have bed rest. I am instructed to avoid travelling until recovery. So I humbly request you to grant me leave for one week. With this letter, I have attached a medical certificate from the doctor whom I consulted for your perusal.
